# Health Questionnaire

**Step 1: Click on Health Questionnaire.**

**Step 2 : Click on the add field.**

**From here you can edit the question you created in the custom field.**

**Step 3: Click on save button.**

**The selected field will be shown to you in the Questionnaire Form.**
